NBA Injury Reporting


ALL injuries (beyond minor scrapes and bruises) must be reported to Newmarket Baseball Association.  All head injuries must be reported no matter how small.  Please complete the NBA Injury form and submit to admin@newmarketbaseball.com.

More serious injuries that may require insurance coverage should be submitted using the Baseball Ontario Injury Report and will be forwarded to the insurance company.
